We welcome new patients at either of our two medical center locations (Murrell’s Inlet, Charleston) for any of our holistic and functional medicine services we provide
New patients will be required to pay a $100 deposit at the time your appointment is being made. Balance of the package cost is collected at first visit.
It is important to note that rescheduling appointments requires 72 HOURS notice via phone 800-965-8482.
New Patient appointments that are missed for any reason or not rescheduled in the 72 HOURS allowable time frame will incur a $300
no show charge.
Patients requiring Superbills in order file Insurance claims will also be given a copy of their chart notes from that visit. Documentation will be available within 48 hours and can be picked up or mailed to you. It is suggested that the patient provide this information with their claim to avoid requests from Insurance Companies for additional information. (this may include copies of associated labs that would already have been provided to you upon review.)
Patients will need to keep copies of all documentation should it be requested by their insurance company in the future. We will no longer provide any of this information directly to Insurance Companies. Patients will be notified by email should we receive a request for information and it will be their choice to provide or not.
A $2.50 documentation fee (per visit) to provide this paperwork will be added to your invoice. (other fees apply for providing copies of full records)
Prescription Refills: Please contact your Pharmacy for all prescription refills. They in turn will contact us as necessary. There may be a fee required by our office to process certain refills. This will need to be collected prior to any refills being processed. Refills may take up to 4 days to process so please plan accordingly.
Office Visit Fee Schedule: Our fees are changing effective immediately. Please call our office for pricing.
New Patients: please inquire and a detailed information package will be sent to you to review your options.
Visit length requirements are based on complexity of what is to be reviewed and is determined by the provider. It is the patient’s responsibility to inquire and be aware of all fees associated with their appointments.
Prices are subject to change without notice at any time. Patients are responsible for understanding fees and their responsibility for such fees. Payments are required at time of service or prior. Deposits and cancellation fees may apply.
